Job Description
As a Home Health Nurse or RN, you will be responsible for traveling to several homes a day to administer healthcare services and help patients maintain their independence. Duties may include administering at home IVs, changing dressings, cleaning wounds, and foley/catheter insertions.
Responsibilities
- Case management and supervised visits.
- Perform skilled and comprehensive assessment of the patient during the initial admission.
- Assess and chart observations of the patient’s condition at each visit (vital signs.
- Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).
- Provide proper patient education for home health care procedures and strategies.
- Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.
- Maintain confidentiality of patient PHI through HIPPA law.
- Keep the patient comfortable through palliative care.
Qualifications
- Current Texas State license as a Registered Nurse.
- 2+ year's experience as a RN in Home Health.
- Current CPR certification.
- Reliable transportation.
- Texas Driver's License and Auto Liability Insurance.
- Current TB Skin Test
- EMR experience (Kinsser preferred).
