Position Summary
Care Professionals provide the maximum quality of care clients while assisting with activities of daily living (ADL’s). Care Choice Home Care’s Care Professionals must adhere to the plan of care and communicate effectively to the Case Manager/Supervisor. Care Professionals may include, but not limited to the following:
Tasks, Duties, Responsibilities
- Understands and utilizes supervisor’s/case manager’s plan of care to ensure high quality patient care.
- Maintains the comfort, privacy, and dignity of patients in the delivery of services to them. Interacts with residents in a manner that displays, empathetic, warmth and promotes a caring environment.
- Advocates for patient.
- Ensure all documentation is complete and accurate, by using the Matrix Care App on your personal device.
- Provide patients with help moving in and out of beds, baths, wheelchairs, or automobiles, transfers and assistance with ambulation.
- Proficiently performs bathing, toileting, diapering, feeding, medication reminders, oral care, skin care, and overall hygiene on male and female clients.
- Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert.
- Prepare or serve meals to patients or other family members, according to prescribed diets as stated in the care plan.
- Perform housekeeping duties, such as changing bed linens, cooking, cleaning, washing clothes, dishes, and running errands.
- Transport clients to locations outside the home, such as to physicians' offices or on outings, using a motor vehicle.
- Observe patients taking medication.
- Performs all job responsibilities in accordance with prescribed safety and infection control procedures and CDC guidelines, including thorough hand washing, use of disposable gloves and proper disposal of soiled materials. Uses safety equipment as directed in the plan of care.

- Effective communication amongst clients and case managers/office staff. Communicate information and ideas in speaking so others will understand.
- Critical thinking- using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems for optimal outcomes.
- Managing one's own time and the time of others.

Pay: $20.00 - $23.00 per hour
